Doctor on Duty at Vaccination Center Decided I am Unsuitable/Unfit for Pfizer and Moderna Covid-19 Vaccines

Today 14 June 2021 Monday I went to Raffles City Convention Center Vaccination Center at 2 PM Singapore Time. I presented the doctor on duty with a long list of 17 medical conditions. The doctor on duty decided I am unsuitable/unfit for Pfizer and Moderna Covid-19 vaccines. Please refer to screenshot of the doctor's memo below.
